PLEASE ALSO USE THIS FORM TO RSVP FOR IN-PERSON ATTENDANCE FOR THE MUKTAD PRAYERS BY GOING TO THE END OF THE FORM.
The contribution towards the costs for prayers is $10.00 per name
Please note the following when filling out the names:
• No surnames are recited in the prayers.
• The name of the deceased is always recited together with his/her father/husband’s name as per Zoroastrian custom.
• The name of the deceased as well as the name of the father/husband is preceded by their prefix, i.e. Ervad, Behdin, Osti etc. Kindly indicate the prefix by selecting the appropriate abbreviation for each name in the drop-down below.

- Ervad (male who is/was a Navar)
- Osta (male from Athornan or priestly family but not a Navar)
- Osti (female from Athornan family)
- Behdin (male/female from Behdin or non-priestly family)
- Khud (whose Navjote has not been performed)
